ADULT DAY PROGRAM FACILITATOR ON-CALL / CASUAL POSITIONS

Haldimand & Norfolk Locations

Program operates Monday-Friday 8:30am-3:30pm

Starting Wage $17.85 (PLUS PSW Wage Enhancement)

Responsibilities:  To assist with the implementation of clinical / therapeutic Adult Day Programs, which actively support adults living with Dementia’s, frail elderly and adults living with disabilities. Related experience in; case management / program planning, facilitating senior’s therapeutic recreation / exercise programs, providing caregiver support and assisting with ADL’s. 
Qualifications: College diploma / certificate in Personal Support Worker or Developmental Services Worker, First Aid/CPR and – 1-2 years related experience. Practical related work experience with Dementia’s and / or cognitively impaired individuals in a client centered therapeutic, social & recreational environment. Certificate in; GPA, Dementia Studies, STEPS Exercise Program would be an asset.
 
Essential Skills: Proven related skills in planning and implementing therapeutic recreation-based programming, excellent interpersonal communication skills, competent user of Microsoft Office & internet, capacity to work independently within a multidisciplinary team.

ADULT DAY PROGRAM MANAGER

Haldimand & Norfolk, ON
Monday - Friday
8:30am - 4:30pm ~ 35 hours per week
We are hiring an experienced and motivated Adult Day Program (ADP) Manager to lead the operations and delivery of our Adult Day Program. This is a key leadership position responsible for the day-to-day management of the program, including supervision of staff and volunteers, program development, client care, and coordination with caregivers and community partners.

The ADP Manager is part of the agency’s leadership team and contributes to
organizational planning, quality improvement, and strategic direction.
Responsibilities:
-Supervise and support ADP staff, volunteers, and students
-Provide training, performance reviews, and schedule coordination
-Lead team meetings and support a positive team environment
-Complete client assessments and review care plans; monitor client well-being
-Ensure safe delivery of meals, personal care, and medication monitoring
-Step in to support the program during staff shortages or emergencies
-Identify community needs and develop or improve program offerings
-Lead quality improvement and evaluate program effectiveness
-Manage client billing, data reporting, and support budgeting and planning
-Build relationships with caregivers, offering support, resources, and case
management
-Support public outreach and help organize fundraising activities
Qualifications:
-Diploma or degree in Therapeutic Recreation (required)
-Minimum 3 years’ experience in a leadership role in community-based
programs or long-term care settings, including managing staff. (required)
-Strong leadership, communication, and team-building skills
-Current First Aid, CPR, and Dementia training and Montessori programming
-Knowledge of local services for seniors and adults with disabilities
-Ability to work independently as well as in team settings
-Ability to maintain confidentiality
 
Requirements:
-Two-Dose COVID-19 vaccination
-Valid Ontario driver’s license and access to a reliable vehicle
-Vulnerable Sector Search Police Clearance
